[Cz-L]Help Translate this document for Jean Weightman

you will find a document (language unknown, but I think it might be
Polish) in a cursive hand that's hard to read, on a page that's more
than a bit indistinct. No-one said this would be easy <g>, but at least
the document can be enlarged if you click on it.

A real test of your sleuthing abilities -- why not see if you can help
